When Does the Death Card Actually Mean… Death?

I'm new to tarot, so go easy on me 😅

I'm working with the Modern Witch deck and keep wondering — has anyone actually seen the Death card mean someone physically dying? I know it's usually about endings or big transformations, but I'm curious about real cases where it was literal.

Did other cards in the spread point that way, or did you lean more on intuition, spirit guides, or outside signs? Any experiences or thoughts would be really helpful.

I'd be on high alert if The Tower and the 9 of Swords showed up together. I haven't actually seen that combo in a spread myself, but honestly, that pairing would really set off alarm bells for me.

Honestly, I usually take Death literally when it comes up around past deaths. Once I read for a pregnant friend and pulled Death with the Queen of Cups. At first we couldn’t tell if it was about the end of her pre-motherhood life — the close of the pregnancy cycle with the Queen as her new maternal self — or if it was pointing to her maternal grandmother passing, with the Queen showing her stepping into a family-connector role.

In the end she felt it was the latter. To me that’s a clear example of Death referring to a past passing.
